Sr. Case Manager–Civic Center Hotel, Community Housing Partnership (San Francisco)
Community Housing Partnership seeks a creative, engaging, and caring professional to provide support services for a recently leased-up housing site and pilot project serving a mix of homeless clients under a Navigation Center model and residents in rent stabilized units at the Civic Center Hotel – Navigation Center. The Sr. Case Manager provides support services and case management to his/her caseload and is responsible for service connection, groups and activities focused on community building, education, or skill development, crisis intervention, and housing retention. All services are provided within the context of harm reduction and within a Housing First model.
